Case Management And Aftercare Planning In Lancaster, CA

Why Aftercare Planning is Important Part Of Drug And Alcohol Rehab

It might be tempting to assume that once you have completed an addiction treatment program like ours you are good to go.  If you have attended our program, you have been in treatment for the past 30-60 days learning important new life skills, coping techniques, new emotional insights, and relapse prevention techniques, which is now the foundation for your new long-term recovery path.  However, addiction is a chronic, relapsing disease that requires a commitment that extends beyond the limits of a rehab program.  This is where the aftercare support comes in. Our aftercare program is the phase of recovery that helps reinforce your newfound sobriety with ongoing outpatient group therapy, alumni activities, support group participation, possibly sober living, and general peer support.

The best time to plan your aftercare program is while you are still in active treatment.  It is too tempting to ignore the need for aftercare once you have completed our rehab program. This is a common mistake that can have devastating effects.  Early recovery is a very tenuous and vulnerable time, as you are adjusting to living without the crutch of your substance.  By making an aftercare plan in advance of discharge, you can smoothly transition into the real world with all the information you need to have a successful sobriety.

What Exactly is Aftercare Planning?

Aftercare support provides critical reinforcements to early sobriety in a variety of ways.  These include:

Weekly group or individual therapy sessions offer ongoing support in the early months of recovery when challenges, temptations, or triggers might threaten your recovery.

Attending 12-step or similar recovery groups provides the important social support that can offer you fellowship and accountability.Sober living provides a drug and alcohol-free supportive home environment that can be an excellent transitional phase between completing rehab and returning to regular life.

Sober living provides a drug and alcohol-free supportive home environment that can be an excellent transitional phase between completing rehab and returning to regular life.

Early recovery is about rebuilding your life, including making new career or academic goals or seeking a job.  Brushing up on interviewing techniques and resume writing can be very helpful for you on your return to the outside world.

Relapse prevention can be continually fine-tuned as you move forward from rehab back into the real world.  Relapse prevention is a proactive process that must be constantly self-monitored.

Alumni activities are an important part of peer recovery. Social activities and follow-up contact with others who have completed the program offers you the important peer support you need.

Meet Case Manager, Krystle Britt, Answers Questions About Case Management..

We talked with Krystle to learn more about her professional experience as the Case Manager at Quest 2 Recovery. Located in Southern California, Quest 2 Recovery is a residential rehab center dedicated to helping people recover from drug and alcohol addiction. Krystle's level of compassion and insight became clear right away as we asked questions about her role in people’s recovery journeys. She shares more information about the kind of assistance they provide as she answers some of the the most frequently asked questions below.

We meet with all of our clients within the first 24 hours of their arrival to get an idea of what their current socioeconomic [financial and living] circumstances are like. The goal of the initial meeting is to establish a baseline of where they are at that moment and to begin to create a treatment plan that is customized to their needs. We also begin a conversation about their aftercare options right away to set them up for success.

We know that most people will not feel well either physically or emotionally , or both, when they walk through that door. They have made a life changing decision and we are here to support in that change. We will always try to meet with them but we will keep things as to the point as possible in that first meeting. We have seen what works so once we know more about the individual, we can ensure treatment is set up to work for them.

After our initial meeting within their first 24 hours of care, and after they have been participating in the inpatient treatment phase, I meet with them again seven days later, and that’s when we really begin the after care planning. At this point  they are feeling better physically and emotionally and kind of have an idea of what their recovery looks like for them. It’s all based on an individual’s needs and circumstances –  together we identify what will help to give them long term success in recovery based on their input and experience and our knowledge and resources.

After seven days of detox and residential treatment, we meet with our clients and go through the different levels of care we offer, and why it’s essential to continue to receive support once they leave our facilities and our residential program.

We will often make a referral to a nearby partial hospitalization program (PHPs) for clients that can benefit from attending one, if circumstances allow.

We will also help them research PHPs in their home state if they prefer.

If a PHP isn’t the right choice, we connect clients with a qualified therapist and/or psychologist in their area that they can meet with regularly to get continued support.

We also create a schedule of AA meetings, Smart Recovery group meetings, and other programs in their hometown that they can attend when they return.

We understand the impact financial hardship has on people’s recovery goals and their ability to access the support they need. Many of our clients come to us in Southern California from other states, and we help them with financial counseling and even getting signed up for food stamps.

Getting started on a sober life can be difficult with the financial consequences of addiction so we continue to our patients develop the necessary tools for success even after they graduate from our program.

For clients that would like to stay in California and attend a partial hospitalization program, we can put them in touch with PHPs that come with housing arrangements. There are quite a few local programs in Palmdale, Quartz Hill, Lancaster and near Edwards Air-Force base that charge a low fee for people experiencing financial hardship.

For clients that would like to stay in California to attend a partial hospitalization program after residential rehab, we can put them in touch with PHPs that come with housing arrangements. There are quite a few local programs in Lancaster, Palmdale, Quartz Hill and Rosemont that charge a low fee for people experiencing financial hardship and some will even allow clients to attend without paying anything upfront with an agreement that they will pay once they are employed.

Our clients are usually out-of-state residents, which is why our recovery program focuses so much on aftercare and alumni support. People often want to get as far away from their current living situation as they can, either because it’s contributing to their substance abuse disorder or because they purposely want to make it harder to leave rehab early. This is why we find out the best option and if that means attending a partial hospitalization program nearer to their hometown we will work to ensure those connections are made and that they have somewhere to go as soon as they get home. It’s really important to us to stay connected to our alumni and no matter where they come from, we follow up with every client to make sure they have the aftercare support they need and are staying on track with attending therapy and support groups.

At Quest 2 Recovery, our holistic addiction treatment philosophy is focused on the unique needs of the individual during inpatient rehab. Our knowledgeable and professional team members offer incredible support and guidance to everyone who attends one of our programs and can also help oversee the practical details involved with attending treatment to make your path to recovery as smooth as possible. With a trauma based approach in a safe, family-like environment, we offer tailored treatment plans to help people learn more about and resolve the underlying issues that cause substance abuse. How Aftercare Planning Can Aid Recovery

Staying Connected Improves Success in Recovery

The post-rehab landscape is full of potholes that can quickly trip up even the most determined individual.  This is why you must be committed to lifelong recovery which involves an aftercare plan.  Especially important in the first several months following treatment, aftercare support can provide the reinforcement you need to build up recovery skills and succeed in sobriety.  Instead of flailing about and trying to manage the multitude of issues that commonly present themselves alone, aftercare provides you with multiple layers of support.

Alumni Program at Quest 2 Recovery in Lancaster, CA

An important aspect of aftercare is the alumni support.  Alumni support is provided by the clinical team at Quest 2 Recovery who will check in with you on a monthly basis following the completion of our addiction treatment program.  Periodic motivational guest speaker events and other alumni events keep you engaged with other alumni in early recovery.

Outpatient Programs for Continued Care in North LA County

Outpatient treatment involves the ongoing therapy that will provide you with emotional support in recovery.  Life is messy.  Once you leave our treatment facility, real life can impose stressors and challenges that, if you do not have a solid support base, can test your ability to stay clean and sober.  Having weekly therapy sessions with a psychologist who can offer you guidance, introduce new coping techniques, help you refine relapse prevention strategies, and provide emotional support if personal struggles emerge is an important asset in recovery.
